Low-grade inflammation and muscular fitness on insulin resistance in adolescents

Resumen

AntecedentesEl bajo estado físico muscular (MF) y la inflamación de bajo grado se han relacionado con la resistencia a la insulina (IR).ObjetivoEvaluar las asociaciones entre la MF y una puntuación agrupada de biomarcadores inflamatorios en la IR e investigar el impacto combinado de la MF y la inflamación en la IR en adolescentes.MétodosEste es un análisis transversal con 529 adolescentes (267 niñas) de 12 a 18 años de edad. Se evaluó el estadio puberal, el estado socioeconómico, la adherencia a la dieta mediterránea, el estado cardiorrespiratorio y la circunferencia de la cintura. Como indicadores de MF se utilizaron la dinamometría de salto de longitud de pie y la dinamometría isométrica de la empuñadura. Se evaluó la puntuación continua de los biomarcadores inflamatorios agrupados (InflaScore) (suma de las puntuaciones Z de la proteína C reactiva, C3, C4, fibrinógeno y leptina) y la IR (evaluación del modelo de homeostasis de la resistencia a la insulina[HOMA-IR] estimada a partir de la insulina y la glucosa en ayunas).ResultadosLa insulina HOMA-IR y la insulina en ayunas se asociaron positivamente con InflaScore y negativamente con la MF, independientemente de la edad, el sexo, el estadio puberal, el estatus socioeconómico, la adherencia a la dieta mediterránea, el estado cardiorrespiratorio y la circunferencia de la cintura. Los adolescentes clasificados como High InflaScore/Unit mostraron un HOMA-IR significativamente más alto cuando se compararon con aquellos con High InflaScore/Fit y con aquellos con Low InflaScore/Fit (F(3,519) = 4.761, P <.003), después de los ajustes para los posibles factores de confusión. Los adolescentes no aptos con un alto índice InflaScore tuvieron las mayores probabilidades de expresar un alto índice HOMA-IR (odds ratio, OR = 2,40; intervalo de confianza[IC] del 95%: 1,2 a 5,6) y riesgo de insulina (2,53; IC del 95%: 1,5 a 5,9) cuando se compararon con los del grupo Low InflaScore/Fit, después de los ajustes para los posibles factores de confusión.ConclusiónLos niveles más altos de MF parecen minimizar el efecto deletéreo de la inflamación en los IR.
