Abstract
There are several alternatives for the management of supraventricular tachycardias, some of them pharmacological and others nonpharmacological. Among the pharmacological ones, sodium channel blocking antiarrhythmic drugs, class IC, such as propafenone, have been shown to be high risk in elderly patients, due to the possibility of precipitating heart failure. This case illustrates a medication error consisting of a problem related to the selection of the antiarrhythmic drug, in which ignoring the alert generated by the pharmaceutical service caused a serious adverse reaction (heart failure), which corresponds to a completely preventable safety problem.
