Description
Inter-institutional collaboration project that contributes to the control and mitigation of Chronic Noncommunicable Diseases (NCDs) to reduce the morbidity and mortality of these conditions through the evaluation of a multimodal intervention to integrate the differential approach in patients with AHT and DM and in providers of primary and complementary health care services of Priority Health Care Centers (CAPS) of the Sur Occidente E.S.E. Integrated Health Services Subnet, with the purpose of strengthening Comprehensive Health Risk Management and the provision of health services.
It uses mixed methods (quantitative and qualitative) that analyze and monitor individual and group actions aimed at their own health care, with patients and professionals, who participate in the control of HBP and DM.
