Relationship between the total maternal mortality ratio and the maternal mortality ratio due to obstetric hemorrhage with the satisfaction of demand for red blood cell units and live births per departmental obstetric services in Colombia between 2018 and 2023

INTRODUCTION: Obstetric hemorrhage is one of the leading causes of maternal mortality worldwide. This study evaluates the correlation between the total maternal mortality ratio (TMMR) and the maternal mortality ratio due to obstetric hemorrhage (MMROH), in relation to the availability of red blood cell units and the proportion of live births in relation to obstetric services at the departmental level in Colombia between 2018 and 2023. The relationship between maternal mortality and out-of-hospital births was also evaluated. MATERIALS AND METHODS: A retrospective cross-sectional study between 2018 and 2023 evaluated cases of total early maternal death and maternal death due to obstetric hemorrhage from the databases and maternal mortality event reports of the Public Health Surveillance System (SIVIGILA). Cases of death from obstetric hemorrhage were divided by the number of live births each year, published by the National Administrative Department of Statistics (DANE), and multiplied by 100,000 to establish the RMMHO. Information regarding obstetric services that transfused patients was obtained from the Hemovigilance Information System (SIHEVI-INS). The comparison between variables was performed using Pearson's correlation. This work was considered exempt from approval by the Institutional Review Board because the data used are public.
RESULTS: Between 2018 and 2023, 1,921 early maternal deaths were reported, establishing an MMR of 53.1. 18.3% were associated with gestational hypertensive disorders, 16.7% were secondary to obstetric hemorrhage, and 6.2% were due to infectious processes. The 271 obstetric services analyzed transfused 59,334 women. No relationship was identified between the MMR and satisfaction of erythrocyte demand (r = -0.087, p = 0.627). Nor was any relationship found between the MMR and live births based on obstetric services reported by department (r = -0.081, p = 0.657). When defining MMR and MMRHO as independent variables with respect to the percentage of out-of-hospital births, we found a statistically significant correlation (r = 0.707, p = <0.001 for MMR and r = 0.7012, p < 0.001 for MMRHO). CONCLUSION: There was no correlation between the increase in RMM and RMMHO and the satisfaction of demand for red blood cell units between 2018 and 2023. Nor was there any association between this increase and the relationship between the number of live births and the number of obstetric services available at the departmental level. However, a direct connection was found between the maternal mortality rate and obstetric hemorrhage and the percentage of out-of-hospital births.
